Use this printable blood pressure log to track your blood pressure and heart rate daily to help with your next medical appointment!

Free printable blood pressure log
I know… I know… this printable isn’t my usual fun and exciting kid friendly version, but that’s ok! Sometimes you just have to suck it up and do adult stuff, like track your blood pressure. I am creeping up on 40 quicker than I want to admit and with age comes all sorts of issues including high blood pressure! My own physician asked me to start tracking my blood pressure and, as the story goes, there had to be a printable for it! Because I was honestly making it for myself anyways haha.
Blood pressure tracking sheet
This blood pressure tracking sheet is basic and to the point, no extra frills or fluff because that’s not always necessary (though it is always fun). With this log you can track the date, time, systolic (upper number), diastolic (lower number), and heart rate as well as any notes that may pertain to that reading.
This is not meant to be a replacement for any medical device or service, the sole purposes of this tracker is so you can track at home and have something to easily take to your next medical appointment!
What’s Included:
8.5×11 PDF printable blood pressure log

Supplies Needed:
Below are the supplies I recommend to get the best out of your new printable!
- Printer (this HP ones my favorite)
- Blood pressure monitor
How to use the log
You’ll first need the supplies listed above, specifically the blood pressure monitor, to start tracking at home. If you’re not familiar with monitoring your blood pressure for yourself I highly recommend checking out this article for home blood pressure monitoring from the American Heart Association.
It is recommended to take your blood pressure a few times throughout the day and track it accordingly, this printable allows you to track as much as you need as the columns are endless. So if you’re asked to track more than twice a day by your physician you can. You can print as many copies as you need for tracking!

Meet Laura Nicholson
I run a full time graphic and web design business but sometimes need my own creative outlet! And what better way than doing what I enjoy most, designing printables!

Thank you for allowing me to download your free blood pressure chart. It is refreshing to be able to get a little help from a perfect stranger that really is free with no strings attached. If I need any graphics I will be in touch and I will tell others about this experience.
PS I want to make up a protest sign and I am considering getting it done professionally. Do you print sheets of graphic material that I could stick onto a piece of foam board?
